Patients in Southampton City have access to a wide range of community health services which aim to promote and improve your physical health and wellbeing. The majority of these services are delivered close to home and where necessary, at home. Local community health services that we commission include:

  • Community Nursing
  • Community Rehabilitation Teams
  • End of Life Care
  • Health Promotion, including the Quit4Life smoking cessation service
  • Inpatient and outpatient services in community hospitals
  • Leg ulcer care
  • Occupational Therapy
  • Orthopaedic Choice Service
  • Physiotherapy
  • Podiatry
  • Rapid Response Teams
  • Specialist Nursing Teams
  • Speech and Language Therapy 

All of these services are delivered by Solent NHS Trust.

Children’s Community Health Services

There are a broad range of services offered for children in the community. Our key priorities are to make sure children’s care, wellbeing and safety are prioritised. We are continually working to ensure care and processes are of a high standard and that they meet our responsibility to safeguard and promote the welfare of children and young people in Southampton. There are many services for children aged from 0 -19 years with services delivered in partnership with other specialist agencies in Hampshire. The services include:

  • Public Health Nursing (Health Visiting and School Nursing)
  • Therapy services (Physiotherapy, Occupational Therapy & Speech and Language Therapy)
  • Community Paediatric Medical Services
  • Community Children’s Nursing
  • Children and Adolescent Mental Health Services (CAMHS)
  • Learning disabilities and complex health conditions
